informed consent for written remote consultations

This document explains, in clear terms, what a written consultation through off.clinic is and what you consent to when you submit a request to a doctor through the platform.

1. nature of the service

  • The consultation is written and asynchronous, based on the patient's submitted information and files.
  • The service is intended for non-urgent situations compatible with a written medical review.
  • The doctor reviews the raw materials you provide directly.

2. what this service does not provide

Important

  • This is not an emergency service. In an emergency, call 112.
  • It does not provide real-time monitoring.
  • It is not appropriate where an immediate physical examination, urgent testing, procedures, or rapid in-person assessment are needed.
  • The platform does not automatically issue prescriptions, sick leave certificates, or autonomous clinical decisions.

3. role of the doctor and AI support features

  • The doctor remains solely responsible for clinical review, triage, and recommendations.
  • If AI support features are enabled, they may assist only with internal drafting or summarisation and do not make medical decisions on the doctor's behalf.
  • The doctor may ask for clarification, redirect you to in-person care, or close the request if this format is not appropriate for your case.

4. what you consent to

  • You consent to off.clinic collecting, storing, and transmitting the data you submit to the selected doctor for handling your request.
  • You consent to the processing of health data, including messages, images, and uploaded documents, to the extent necessary for this consultation.
  • You understand that the doctor may direct you to clinic or emergency services if clinically required.

5. consultations for minors

  • For pediatric consultations, the patient is the child, and the account holder is the parent or legal representative who requests the consultation on the child's behalf.
  • A request may only be made by a person aged at least 18 who declares that they are the parent or legal representative of the child and have the authority to act on the child's behalf.
  • The legal representative consents to the processing of the child's health data (special-category data under Article 9 GDPR) for the purpose of this consultation.
  • The resulting medical document is issued in the child's name, noting that it was requested by the legal representative.
  • Where deemed necessary, the doctor may request within the consultation a document evidencing the legal-representative status (for example, a birth certificate); you can provide it as an attachment through the platform.

6. before you continue

Continue only if you understand that this is a non-urgent written service, without real-time monitoring, and that the doctor may decide an in-person assessment or urgent escalation is needed.

The account holder must be at least 18 years old. For a minor, the request may only be submitted by the parent or legal representative, under the conditions in section 5.
